A Reminder For Our Patients

- Please bring a valid health card with you when coming to see a physician or a nurse. If invalid, lost or forgotten, a reimbursable $25.00 + HST fee may apply.

- Appointment cancellations must be made no less than 24 hours prior the scheduled appointment or no-show fees may apply.

- We now offer email confirmation for booked appointments. Please ensure we have your current email address. We are not able to provide appointment-reminder phone calls.

- If you are coming in for a Periodical Health Review or prenatal appointment, please arrive 15 minutes prior to your appointment.

- During the rainy days and winter months, outdoor shoes and boots are not allowed in the clinic including the waiting room area. Please leave them in the lobby on the dedicated racks.

Please Note: Hog's Back swing bridge swings in the summer when boats exceeding 9 feet in height need to pass. This action can block the traffic in that area for approximatively 15 minutes. Please take into consideration when planning your visit at MFHC. If you miss your appointment due to this, we will not be able to accomodate your late arrival.

DO NOT COME TO THE CLINIC IF YOU HAVE ANY OF THE FOLLOWING SYMPTOMS:

- Fever

- Cough SOB

- New or Worsening Acute Respiratory illness

- Sore throat, Runny nose, Nasal congestion, Hoarse voice, Difficulty swallowing

- If you have had contact with Covid positive people or suspected cases that you know of

- Headache, loss of taste/smell chills, abd pain, nausea, vomiting, diarrhea, unexplained fatigue

If you wish to book an appointment with a nurse, please call their line at 613-228-1950 and leave a message, they will call you back to book the appointment. We are no longer accepting drop ins for nurses, ALL appointments MUST be booked in advance.

WE ARE NO LONGER OFFERING ALLERGY SHOT SERVICES UNTIL FURTHER NOTICE.

In order to help reduce the spread of the virus - we ask for your help. If you have respiratory symptoms (cough, cold, fever, runny nose, sore throat), have travelled, or have had contact with someone with respiratory symptoms and travel, we ask that you STAY HOME and contact Public Health https://www.ottawapublichealth.ca 613-580-6744 for further direction. PLEASE DO NOT ATTEND OUR OFFICE WITHOUT INSTRUCTION IN THIS CASE.

As a community, our responsibility at this time, is to minimize the virus spread and the impending impact on our healthcare system. If you are able to delay your elective appointments, we urge you to do so. We also ask that you do not attend the EMERGENCY ROOM unless it is a clear EMERGENCY. There are other modes of testing if you are concerned you have contracted the virus.

In regards to your greater social obligations - we urge you to wash your hands and just stay home. Please reconsider attend large public venues, events and travelling. The more successful we are at social distancing, the best chance we have at minimizing the impact of this virus.
